Alchemy Massage LLC ("we," "us," "our") operates alchemymassageohio.com and provides massage therapy services in Heath, Ohio. This policy explains what information we collect, why we collect it, who we share it with, and what choices you have.

WHAT THIS POLICY COVERS

This policy covers information collected through our website, our online booking system, member accounts, our loyalty and referral programs, and gift card purchases. It also covers the health intake information you give us in connection with a session.

A NOTE ABOUT HIPAA

We do not bill health insurance, so we are not a "covered entity" under HIPAA, and this policy is not a HIPAA Notice of Privacy Practices. That does not lower our duty of care. We treat your health information as confidential, we do not sell it, and we use it only to serve you safely.

INFORMATION WE COLLECT

When you book a session. Your name, email address, phone number, appointment date and time, and your appointment history with us.

Health intake information. Before or during your first session we ask about your health - conditions and diagnoses, injuries, surgeries and recovery status, medications, pregnancy, areas of pain or swelling, and anything else that could make massage unsafe for you. We collect this for one reason: to decide whether massage is appropriate for you, to adapt the session, and to avoid harm. We treat it as the most sensitive information we hold.

When you create a member account. Your login details and any preferences you save.

Loyalty and rewards. Your points balance, visits, and qualifying purchases.

Referral program. If you refer a friend, we collect the name and contact details you give us for that person. We use them to send that person one message about your referral. We do not add them to any marketing list unless they ask us to.

Gift cards. The purchaser's and recipient's names and email addresses, and the purchase record.

Payments. Card payments are processed by our payment provider. We do not receive or store your full card number.

Automatically, when you visit the site. IP address, browser and device type, pages viewed, and how you arrived. This is collected by our website platform and analytics tools, described below.

HOW WE USE IT

To schedule, confirm, and remind you about appointments. To screen for contraindications and deliver your session safely. To take payment and keep business records. To run the loyalty, referral, and gift card programs. To answer your questions. To send marketing or newsletters, only if you have opted in - every message has an unsubscribe link. To understand how the website is used, so we can improve it. To meet legal, tax, and professional obligations.

WHO WE SHARE IT WITH

We do not sell your personal information, and we do not share your health intake information with anyone for marketing.

We share information with service providers who help us run the business: Wix, for website hosting, online bookings, forms, member accounts, and email; our payment processor, to take and record payments; Google Analytics, for website traffic measurement; our accountant, for financial records; and anyone we are legally required to share with, for example in response to a valid legal request.

We use Google Analytics to understand website traffic. You can read how Google uses data from sites that use its services at policies.google.com/technologies/partner-sites, and you can opt out using Google's browser add-on at tools.google.com/dlpage/gaoptout.

COOKIES AND TRACKING

Our website uses cookies and similar technologies - some necessary for the site and booking system to work, and some to measure how the site is used. Most browsers let you refuse or delete cookies. If you do, parts of the booking system may not work.

Do Not Track. Some browsers can send a "Do Not Track" signal. There is no common industry standard for how to respond to these signals, and our website does not currently respond to them.

Third-party tracking across sites. Our website platform and our analytics provider may collect information about your visit to this site. Google Analytics may also collect information about your activity across other websites that use Google's services.

HOW LONG WE KEEP IT

Health intake and session records: five years after your last visit, and longer for clients who were minors at the time of service. Ohio does not set a retention period for massage therapy records, so this is our own policy, chosen with Ohio's medical-claim limitation periods in mind.

Booking and payment records: as long as needed for tax and accounting purposes. Marketing lists: until you unsubscribe. Website analytics: according to our analytics provider's retention settings.

YOUR CHOICES AND YOUR RIGHTS

You may ask what we hold about you and ask for a copy; correct anything that is wrong; ask us to delete information we are not required to keep; opt out of marketing at any time, using the unsubscribe link or by contacting us; and close your member account.

Your Ohio records rights. Ohio law gives clients of licensed health care practitioners the right to examine and receive a copy of their records on written request. Contact us and we will explain what we hold and how to get a copy.

SECURITY

We use the security controls built into our website and booking platform, we limit who can see client records, and we keep paper intake forms secured. No system is perfectly secure and we cannot guarantee absolute security - but if a breach affecting your information occurs, we will notify you as required by law.

CHILDREN

Our services and website are intended for adults. We do not knowingly collect information from children under 13 through the website. Sessions for minors must be booked and consented to by a parent or legal guardian.

CHANGES TO THIS POLICY

If we make material changes, we will post the updated policy here with a new effective date, and where appropriate we will notify clients by email. Please check back from time to time.
